This online form can be used to request a routine appointment. Routine appointments are released each Wednesday, commencing 17th Septmeber 2025 at 10am. You can submit this form from 7am on a Wednesday morning and you will be sent a text message or receive a phone call, detailing when your appointment will be.

This form MUST NOT be used for clinically urgent appointment requests, urgent prescription requests or life-threatening emergencies. 111 is available in such cases and when we are closed.
